Penggantian/Perpanjangan Paspor Biasa - PPI Wageningen

Syarat-syarat

  • Membawa Paspor lama yang masa berlakunya kurang dari 1 (satu) tahun.
  • Mengisi dan menandatangani formulir permohonan dengan jelas dan lengkap.
  • Surat keterangan (asli dan terbaru) terdaftar sebagai penduduk di suatu Gemeente dengan menyebutkan kewarganegaraannya (International Uittreksel van bevolkingsregister met vermelding v.d. nationaliteit).
  • Foto copy kartu ijin tinggal/ID Card (verblijfvergunning) yang masih berlaku.
  • Foto copy akte kelahiran.
  • Foto copy akte perkawinan (bagi yang telah menikah)
  • Biaya pembuatan paspor baru 50 euro (sudah termasuk biaya foto)

Pemohon harus datang sendiri ke KBRI untuk membubuhkan tanda tangannya dihadapan petugas.

Keterangan tambahan:

  • Jika paspor lama hendak diminta kembali, agar menyerahkan foto copy paspor lama yang telah dicabut dan dibatalkanoleh KBRI.
  • Untuk keseragaman pembuatan pas foto dalam rangka pembaharuan paspor dilaksanakan di KBRI Den Haag
  • Berdasarkan Surat Direktur Jenderal Imigrasi Departemen Hukum dan HAM, No. F-PL.03.10-630 tanggal 23 Maret 2005 perihal penggunaan/penggantian Paspor baru dapat diterbitkan kepada setiap perorangan / individual termasuk anak-anak diterbitkan secara terpisah.
